Player to complete Registration form in ink
Club Officer (Chairman, Secretary or Treasurer) to witness
For clubs with ONE team, player to provide 1 passport size photographs to be signed by player
For clubs with TWO teams, player to provide 2 passport size photographs to be signed by player
For clubs with THREE teams, player to provide 3 passport size photographs to be signed by player
Submission to Registration Secretary must include suitable proof of identity (driving licence with photograph, passport, security pass with photograph).
Player Identity card(s) will be sent to Club Secretaries by League Registration Secretary ( or arrangements can be made for collection)

Each member to show ID to opposition official prior to kick off or players involvement in the match
League Committee Members will be checking ID Cards on Match Days

If player without card participates in a match, he will be deemed an ineligible player.
Match Day Dispute. The team who makes and objection should notify the referee of the objection before the player concerned takes part in the match. The referee will note the objection on his match return card to the League.
The club making the objection MUST notify the league in accordance with League Rule 15.

A Transfer form should be completed and submitted to Registration Secretary together with the£10.00 transfer fee, players current registration card(s) an additional photograph(s) depending upon number of teams.
A player will only be deemed 'transferred' on receipt of NEW identity card.

For any suspension imposed on a player in excess of 7 days, the players identity card(s) MUST be returned to League Registration Secretary within 3 days of commencement of suspension.
Id cards will be returned to Club Secretary upon conclusion of the suspension.
